Breathwork and Cortisol: What the Controlled Studies Actually Show

Breathwork has become one of the most heavily marketed stress-relief practices online, with claims ranging from modest to dramatic: a few minutes of box breathing or cyclic sighing, proponents say, can measurably drop cortisol on demand. The underlying idea, that controlling the breath can influence the stress response, has real science behind it. The specific claim that a short daily routine reliably lowers cortisol is better supported for some practices and study designs than others, and it is worth separating what has been directly measured from what is inferred or extrapolated in marketing copy.
The strongest evidence for breathwork's effect on stress comes from a 2023 meta-analysis in Scientific Reports, which pooled 12 randomized controlled trials covering 785 adults. It found breathwork practices were associated with lower self-reported stress than control conditions, with additional, smaller reductions in anxiety and depressive symptoms across the included studies. That is a meaningful result, but it is worth noting what it actually measured: most of the pooled trials relied on psychological stress questionnaires, not blood or saliva cortisol assays. Feeling less stressed and having measurably lower cortisol are related but not identical outcomes.
Direct cortisol evidence does exist, though it comes from a narrower set of studies with a different practice than what usually gets marketed as a five-minute reset. A 2017 study in Frontiers in Psychology, led by researcher Xiao Ma, put 40 adults through an 8-week, 20-session diaphragmatic breathing program using real-time feedback and a slow target rate of about four breaths per minute. Compared with a no-intervention control group, participants who completed the program showed significantly lower cortisol levels afterward, along with improved sustained attention and reduced negative affect. That is genuine, controlled evidence that a breathing practice can lower cortisol, but it describes two months of structured training, not a single viral technique performed once before a stressful meeting.
This is where the trend and the evidence tend to diverge. Popular short-form content often implies that any slow-breathing technique, performed briefly, produces an immediate, reliable cortisol drop. The research supports a related but more modest claim: sustained, repeated practice over weeks is associated with lower cortisol and improved stress measures in controlled studies, while the acute, single-session effect on cortisol specifically has been studied far less than the acute effect on subjective calm or heart rate variability.
The plausible mechanism connecting breath control to the stress response is well established, regardless of the cortisol question. Slow, controlled breathing activates the parasympathetic nervous system, the body's counterbalance to the fight-or-flight response, largely through its effect on vagal tone. Cleveland Clinic and Harvard Health both describe this pathway as a legitimate, physiologically grounded reason breathing exercises can help shift the body out of an activated stress state, separate from whatever is happening to circulating cortisol at any given moment.
Put together, breathwork sits closer to the "reasonably supported" end of the site's usual spectrum than most trending stress interventions, but with an important caveat about dose and duration. What's solid: slow-paced breathing practices, sustained over weeks, are linked to lower self-reported stress and, in at least one controlled trial, measurably lower cortisol. What's overstated: the idea that a single short breathing session delivers the same effect, a claim the current research has not directly tested. For anyone trying breathwork for stress, that distinction matters more than which specific technique they choose.
