Cortisol 101: What This Hormone Actually Does in Your Body

Cortisol gets talked about online almost entirely in negative terms, as something to "lower" or "balance." That framing skips a basic fact: cortisol is not a toxin your body accidentally produces under stress. It is an essential hormone, made every single day, that your body needs to function at all.

Cortisol is produced by the adrenal cortex, part of the small adrenal glands that sit above each kidney, under instructions from what physiologists call the hypothalamic-pituitary-adrenal axis, or HPA axis for short. In simple terms, the hypothalamus (in the brain) signals the pituitary gland, which signals the adrenal glands, which release cortisol into the bloodstream. This chain exists to help the body manage stress, but "stress" in this context is a broad category. It includes psychological pressure, but also physical stress like illness, exercise, low blood sugar, or simply waking up in the morning.

Under normal, healthy conditions, cortisol follows a predictable daily rhythm: it peaks shortly after waking, then gradually declines over the course of the day, reaching its lowest point around midnight. This is not a malfunction, it is the system working correctly, and it is one of the main reasons sleep schedule consistency matters so much for hormone regulation in general.

Cortisol's job list is longer than most people expect. It helps regulate blood sugar and metabolism, supports blood pressure, and plays a central role in modulating the immune system's inflammatory response. That immune-regulating function is part of why chronic, poorly-regulated cortisol signaling has been linked in recent research to a wider range of downstream effects than the skin alone: reviews have connected HPA axis dysregulation to effects across the neuroimmune, endocrine, cardiovascular, gastrointestinal, and integumentary, meaning skin, systems.

Problems tend to arise not from cortisol existing, but from the system staying activated for too long, or losing its normal rhythm. Chronic activation is associated with glucocorticoid receptor resistance, a state where cells become less responsive to cortisol's normal regulatory signals, which can paradoxically increase inflammation rather than dampen it. This is a meaningfully different picture than "high cortisol equals bad skin," and it is part of why single-cause explanations for skin changes are usually incomplete.

None of this requires alarm about everyday stress. The HPA axis is built to activate and then return to baseline. The research interest, and the legitimate health concern, is specifically about what happens when that return-to-baseline process is chronically disrupted, whether by ongoing psychological stress, irregular sleep, or other sustained physical stressors. Understanding that distinction is the foundation for evaluating every other claim in this category, including the ones about your skin.
