Inflammaging: What Chronic Low-Grade Inflammation Actually Does to Aging Skin

"Inflammaging" is a term gerontology researchers coined to describe a specific, measurable phenomenon: the chronic, low-grade inflammation that tends to accumulate with age, distinct from the short, useful inflammation your body produces after an injury or infection. It's an established concept in aging research, not a skincare marketing invention, though the word has since been borrowed heavily by the skincare industry. Skin, as the body's largest organ and one that's constantly exposed to UV light, pollution, and other stressors, is one of the tissues where inflammaging has been studied most closely.

The mechanism researchers describe involves several overlapping processes. As cells age, some stop dividing but don't die off, instead entering a state called senescence. Senescent cells accumulate over time and secrete a mix of inflammatory signaling molecules collectively known as the senescence-associated secretory phenotype, or SASP. Combined with the gradual decline in the body's ability to regulate immune signaling, this produces a persistent, low-level elevation of inflammatory markers like interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-alpha), and interleukin-1 beta (IL-1beta), even in the absence of any acute illness or injury.

In skin specifically, this chronic inflammatory signaling is associated with several of the visible changes people describe as aging: reduced collagen production, a breakdown of existing collagen and elastin fibers by inflammation-triggered enzymes, and a weakened skin barrier that recovers more slowly from everyday damage. Recent research has also pointed to the skin's own resident immune structures, sometimes called the skin inflammasome, as a driver of this process, and to imbalances in the skin's microbiome as something that may perpetuate it further, though that specific connection is still an active area of study rather than settled fact.

This is also one of the more concrete places where stress research and skin-aging research overlap. A 2024 exploratory clinical study published in the Journal of Cosmetic Dermatology followed subjects under chronic moderate psychological stress and found measurably reduced antioxidant capacity in their skin, along with visible changes in skin texture, compared with less-stressed subjects. That fits the broader picture: psychological stress is associated with HPA axis dysregulation, which over time can blunt cortisol's normal anti-inflammatory signaling and allow inflammatory cytokines to run more freely, essentially feeding the same inflammaging pathway that is driven mainly by cellular aging and sun exposure.

What's genuinely established here is the biology: chronic low-grade inflammation is a real, measurable contributor to skin aging, and it has been a serious research focus for aging scientists for over two decades. What's marketing rather than science is the growing list of skincare products now labeled "anti-inflammaging," most of which have not been tested in controlled studies against the specific senescence and cytokine pathways researchers are describing. A soothing, barrier-supporting moisturizer may genuinely help with comfort and hydration, but that is a different claim than reversing inflammaging at the cellular level, and no topical product has been shown to do the latter.

The practical takeaway lines up with unglamorous, well-supported advice rather than a new product category: daily sun protection, not smoking, adequate sleep, and general stress management are the interventions with the most consistent evidence for slowing age-related inflammatory processes in skin. None of that is new information, but it is worth knowing that "inflammaging" describes a genuine, decades-old area of aging research, which makes it easier to tell the difference between that established science and a label slapped on a jar to sell it.
