Intermittent Fasting and Cortisol: Does It Help or Backfire?

Intermittent fasting, whether as a daily 16:8 eating window, alternate-day fasting, or longer multi-day protocols, is usually marketed around weight and metabolic health. Increasingly, though, it is also pitched as a way to "reset" cortisol and, by extension, calm stress-related skin issues like breakouts or dullness. The actual controlled research on fasting and cortisol specifically is smaller and messier than that pitch implies, and it does not point in one consistent direction.
The most direct evidence comes from a 2023 study in the American Journal of Physiology-Endocrinology and Metabolism, which put 22 healthy volunteers through a 10-day high-calorie period followed by a 10-day complete fast, with cortisol measured at each stage. Fasting increased morning free cortisol and the free-to-total cortisol ratio, the opposite of what "cortisol-lowering" marketing would predict, but the effect was sex-dependent: it was statistically significant in men and essentially absent in women in the same study. That is a meaningful complication for any blanket claim about what fasting does to stress hormones, since it suggests the same protocol may not affect everyone the same way.
Shorter, more Ramadan-style fasting patterns, meaning a daily fasting window sustained over about a month, have been studied somewhat more. A 2020 prospective study in PLOS ONE followed 57 overweight and obese adults through 28 days of diurnal fasting during Ramadan and found significant shifts in several appetite and sleep-related hormones, including ghrelin, leptin, and melatonin. For cortisol specifically, the research literature on Ramadan fasting has more often reported a disruption of the normal diurnal cortisol rhythm, rather than a simple, clean lowering of cortisol, with the timing of the daily peak and trough shifting alongside the altered eating and sleep schedule.
This distinction between "changes cortisol" and "lowers cortisol in a good way" matters, because the two get conflated constantly in wellness content. What the research supports is that fasting is a real physiological stressor capable of measurably shifting cortisol and its daily pattern; what it does not support is a consistent, one-directional "cortisol reset" that applies the same way to a man doing a 16-hour daily fast as it does to a woman doing the same thing, or to someone attempting a multi-day extended fast.
The skin-specific claim is thinner still. No controlled trial has measured whether intermittent fasting's effect on cortisol translates into fewer breakouts, calmer rosacea, or any other measured skin outcome; that leap is inferred from the cortisol-and-skin relationship discussed elsewhere on this site, not demonstrated directly for fasting itself. Given that at least one well-controlled study found fasting raised cortisol in men rather than lowering it, treating fasting as a reliable "cortisol fix" for skin is running well ahead of what has actually been tested.
None of this means intermittent fasting is harmful for most healthy adults who tolerate it well; its metabolic benefits are studied separately and are a different question. But specifically as a cortisol-lowering or stress-skin strategy, the evidence is early, sex-dependent, and sometimes points the opposite direction from the marketing. Anyone with a history of disordered eating, menstrual irregularities, or high baseline stress may want to discuss extended or aggressive fasting protocols with a doctor before assuming it will calm their hormones rather than add to the load on them.
