Perimenopause, Cortisol, and Skin Changes: What's Actually Connected

Perimenopause conversations tend to focus almost entirely on estrogen: hot flashes, irregular cycles, mood swings, and eventually, thinner or drier skin. But estrogen does not decline in isolation. It moves within a genuinely interconnected hormonal system that includes cortisol, the hormone driving most of the "cortisol face" conversation on this site, and that overlap has real implications for skin during this transition.
The hypothalamic-pituitary-gonadal axis, which governs estrogen and progesterone, and the hypothalamic-pituitary-adrenal axis, which governs cortisol, share regulatory circuitry in the brain. As estradiol and progesterone become more erratic during perimenopause, that fluctuation appears to affect HPA axis function as well, not just reproductive symptoms. A 2020 study in the journal Menopause followed 101 perimenopausal women for 12 weeks and found that those with more frequent and severe hot flashes showed a blunted cortisol awakening response, a measurable marker of altered HPA axis activity, even after accounting for hormone levels and sleep quality.
Estrogen also has a direct, well-established role in skin structure. It binds receptors on dermal fibroblasts, the cells responsible for producing collagen, and supports the skin's blood supply and moisture retention. A landmark 1987 study in the British Journal of Obstetrics and Gynaecology found that skin collagen content drops sharply in the years immediately following menopause and continues declining afterward, a pattern that hormone replacement therapy was shown to slow. A 2025 review in the Journal of Cosmetic Dermatology reaffirmed this connection, describing thinner, drier, less elastic skin with reduced barrier function as a consistent feature of the menopause transition, distinct from ordinary chronological skin aging.
Where cortisol comes in is less as an independent cause and more as a compounding factor. Chronic or dysregulated cortisol signaling is separately associated with a weakened skin barrier and worsened inflammatory skin conditions, a connection covered elsewhere on this site. If perimenopause is a period when HPA axis regulation is already somewhat destabilized, layered on top of estrogen's declining direct effects on collagen and barrier function, it is plausible the two systems interact rather than act independently. That said, research testing that combined pathway in skin specifically, rather than in hot flashes or mood, is still limited, and the connection should be read as a plausible mechanism rather than a settled one.
This is also where the science and the marketing start to diverge. That estrogen decline reduces collagen production and skin hydration during perimenopause is well established. What is much shakier is the growing category of products marketed specifically to perimenopausal women promising to fix "cortisol-driven" skin aging through a supplement, serum, or tea. Most of these lean on the general, real HPA-axis changes described above without offering evidence that their specific product changes cortisol, let alone skin outcomes, in perimenopausal women specifically.
None of this is an argument against taking perimenopausal skin changes seriously; they are real and hormonally driven. It is a case for being precise about which hormone is doing what. Hormone replacement therapy has actual trial data behind it for skin collagen and is worth discussing with a doctor for those experiencing significant symptoms. A cortisol-focused supplement marketed around a skin claim built mostly on menopause's estrogen story is a different, far less substantiated proposition.
