Acupuncture and Cortisol: Does It Actually Lower Stress Hormones?

Acupuncture has become one of the more common recommendations in stress-management content, often marketed specifically as a way to "lower cortisol" or "reset the adrenals." The practice itself comes from traditional Chinese medicine and involves inserting thin needles at specific points on the body, traditionally described in terms of qi and meridians. Modern research mostly sets that framework aside and instead asks a narrower, testable question: does acupuncture measurably change stress-hormone output, and does it reduce how stressed people feel?

On the broader question of stress and anxiety, there is real, if modest, evidence behind acupuncture. The National Center for Complementary and Integrative Health (NCCIH) points to a 2014 meta-analysis of 14 studies and over 1,000 participants that found a statistically significant effect of acupuncture over placebo or no treatment for reducing preoperative anxiety. NCCIH also notes reasonable evidence for acupuncture in certain pain conditions, including back, neck, and knee pain, where effects have been comparable to nonsteroidal anti-inflammatory drugs in some trials. That is a meaningfully different, and better supported, claim than "acupuncture lowers cortisol."

The cortisol-specific evidence is thinner and more mixed. In a 2020 randomized-controlled pilot trial published in PLOS One, sometimes called the AkuRest study, 70 adults with elevated stress were assigned to real acupuncture, sham acupuncture, or a waitlist control, with researchers measuring both self-reported stress and morning urinary cortisol and cortisone. Both real and sham acupuncture reduced self-reported stress more than the waitlist group, but the difference between real and sham acupuncture was not statistically significant, a pattern seen in several other acupuncture trials across conditions. That does not mean acupuncture does nothing, but it does complicate any claim that needle placement itself is what drives a cortisol change.

Some of the more detailed mechanistic evidence comes from animal research. A 2014 study in Evidence-Based Complementary and Alternative Medicine, using a rat model of chronic stress, found that acupuncture normalized several components of an overactive HPA axis, including glucocorticoid receptor expression in the hypothalamus and pituitary gland and levels of CRH and ACTHR, the receptor for adrenocorticotropic hormone. Findings like this describe a plausible biological pathway, but rodent HPA-axis studies do not automatically translate into a proven, reliable effect in humans; a separate systematic review of cortisol-specific acupuncture trials found that several randomized controlled trials reported no clear difference in cortisol levels between treatment and control groups.

This is where the gap between the research and the marketing widens. It is fair to say acupuncture has reasonable evidence behind it for certain kinds of pain and some anxiety symptoms, and a plausible biological rationale for interacting with the HPA axis. It is not fair to say, as some wellness content does, that acupuncture reliably "detoxes cortisol" or fixes "adrenal fatigue," a term that does not correspond to a diagnosis recognized by the Endocrine Society or any major medical body. Subjective stress relief from an acupuncture session, including the calming ritual of lying still for 30 to 45 minutes, is real and worth something on its own, even without a measurable hormone change behind it.

If you are curious about acupuncture for stress, the current evidence supports trying it as a reasonably low-risk complementary option, ideally with a licensed practitioner, rather than as a substitute for addressing the underlying sources of chronic stress or for medical evaluation of a suspected hormonal condition. Anyone with a bleeding disorder, a pacemaker, or who is pregnant should talk to a doctor before starting, and NCCIH's safety guidance is a reasonable starting point for those questions.
