Caffeine and Cortisol: Does Your Morning Coffee Actually Raise Stress Hormones?

Caffeine gets folded into the "cortisol" conversation constantly, usually as a warning: that morning coffee is supposedly spiking your stress hormone and undoing whatever else you are doing to keep cortisol in check. The physiology behind that claim is real, but the size and meaning of the effect are usually overstated.
The clearest evidence comes from a controlled crossover trial published in Psychosomatic Medicine, which had 96 healthy adults abstain from caffeine for five days, then gave them capsule doses of 0, 300, or 600 milligrams. Caffeine did trigger a real, measurable rise in both ACTH, the pituitary hormone that signals the adrenal glands, and cortisol itself, the same "stress axis" pathway activated by a hard workout or an argument. A more recent comparative review presented at a 2025 joint endocrinology congress found coffee produced the largest relative cortisol rise among caffeinated drinks, consistent with its higher caffeine content compared with tea.
The detail that usually gets left out of "coffee spikes cortisol" headlines is tolerance. In that same crossover trial, five days of regular caffeine intake blunted the cortisol response to a first dose of the day, though it did not eliminate it entirely, and a rise still appeared later after a second dose. In other words, if you drink coffee daily, your body has already adapted somewhat to that specific stimulus. The dramatic spike sometimes described online applies more to an occasional or caffeine-naive drinker than to someone with an ordinary daily habit.
Timing is where the science gets thinner and the trend content gets louder. Wellness content often claims you should delay your first cup by 90 minutes because cortisol is naturally highest right after waking and adding caffeine on top "overloads" the system. That idea has some physiological logic, since cortisol does follow a predictable daily peak-and-decline pattern, but it has not been tested in a rigorous trial that shows meaningfully better outcomes from delaying coffee specifically. It is a reasonable, low-cost thing to try if you are curious, not an established recommendation.
It is also worth being clear about what an acute, caffeine-induced cortisol rise does and does not mean. A transient increase after one cup of coffee is not the same physiological event as chronic, poorly regulated cortisol exposure, which is the pattern actually associated with disrupted sleep, skin barrier changes, and inflammatory flares elsewhere on this site. No controlled study has shown that ordinary coffee consumption, at typical daily intakes, produces the sustained HPA axis dysregulation linked to those downstream effects. Treating a normal cup of coffee as equivalent to chronic stress overstates what the caffeine research actually shows.
The practical takeaway lines up with general caffeine guidance rather than any cortisol-specific rule: Mayo Clinic guidance puts roughly 400 milligrams a day, about four cups of brewed coffee, as a reasonable upper limit for most healthy adults. If you are especially sensitive to caffeine, sleep poorly, or notice jitteriness or anxiety after coffee, cutting back is reasonable self-management, but it is not the same as coffee being a proven driver of chronically elevated cortisol. For most people with an ordinary intake, the daily cup is a minor, transient, and well-tolerated blip on an otherwise normal hormonal rhythm.
