Cold Plunges and Cortisol: Do Ice Baths Actually Lower Stress Hormones?

Cold plunging has gone from a niche athletic recovery tool to a mainstream wellness ritual, and cortisol sits at the center of how it gets marketed: sit in near-freezing water for a few minutes and, the claim goes, your stress hormone drops. The physiology involved is real, but it is more layered and delayed than that pitch suggests once you look at the actual controlled research.

Physiologically, sudden cold water exposure is itself a significant physical stressor. It activates the sympathetic nervous system and, in the short term, is expected to raise rather than immediately lower circulating stress hormones, as part of the body's normal cold-shock response, before any calming effect appears. This detail is often missing from cold plunge marketing, which tends to describe cortisol reduction as instant rather than as something that shows up later, well after the initial stress response has resolved.

The most direct controlled evidence on this timing comes from a 2023 study in the Journal of Thermal Biology. Sixteen healthy adults completed a single 15-minute session of cold water immersion at about 50°F, with blood samples taken before, during, and after. Serum cortisol was not different from baseline immediately after immersion, but it was measurably lower three hours later, alongside a corresponding drop in self-reported negative mood. That is a real, published effect, though the study was small and single-session, and it only tracked outcomes three hours out, so it says little about what happens with repeated, long-term plunging or under less controlled real-world conditions.

A larger 2025 systematic review and meta-analysis in PLOS ONE pooled 11 studies covering more than 3,000 participants and found a similar delayed pattern: a significant reduction in perceived stress emerged around 12 hours after cold water immersion, but no significant effect on stress was detected immediately, at one hour, at 24 hours, or at 48 hours post-exposure. Combined with improvements the review found in sleep quality and general wellbeing, this points toward cold water immersion having a real but narrow, delayed window of stress-related benefit, rather than the instant reset implied by most social content.

None of this current evidence says anything specific about skin. The "cortisol face" framing that connects cold plunges to skin appearance is an extrapolation from the stress-hormone research, not a claim tested in its own right; none of the cited studies measured skin outcomes directly. It is also worth being clear about risk. Cleveland Clinic guidance cautions that cold water immersion carries real risks for people with heart disease, high blood pressure, or circulation problems, and recommends beginners start with water in the 50 to 59°F range for no more than three to five minutes, rather than jumping straight to the more extreme temperatures shown in viral videos.

The honest summary sits between hype and dismissal. Proven: a single session of cold water immersion is associated with measurably lower cortisol and improved mood, but only hours afterward, not in the immediate glow influencers often show on camera. Genuinely supported by a sizable pooled analysis: cold water immersion is linked to reduced perceived stress and better sleep quality, though on a similarly delayed timeline. Not supported: any specific claim that cold plunging fixes "cortisol face" or produces an instant hormonal reset. For most healthy adults, a brief, appropriately cold plunge is a reasonable stress-management tool to try alongside sleep, diet, and other established habits, not a replacement for them.
