Epsom Salt Baths and Cortisol: Does Magnesium Actually Absorb Through Skin?

Epsom salt baths are one of the oldest wellness rituals still getting a modern rebrand, now marketed specifically as a cortisol-lowering habit: soak in magnesium sulfate dissolved in warm water, the pitch goes, and magnesium absorbs through your skin, replenishes a mineral tied to stress regulation, and brings cortisol down. It is a compelling story, partly because magnesium’s role in stress physiology is genuinely real and well established in the nutrition literature. Whether it can get into your bloodstream in meaningful amounts by sitting in a bathtub, rather than through what you eat or swallow, is a separate, much shakier question.
Magnesium is a cofactor in hundreds of enzymatic reactions and plays a role in regulating the HPA axis, the hormonal pathway that governs cortisol release, and some research has associated lower magnesium status with higher markers of stress and inflammation. That is the legitimate physiology behind the marketing, and it is a large part of why magnesium supplements are so widely recommended for sleep and stress. But that research base is almost entirely about oral magnesium intake or supplementation, not about soaking in it, and the two are not automatically interchangeable claims just because both involve the same mineral.
A 2017 review in the journal Nutrients, examining the existing literature on transdermal magnesium, found the evidence for meaningful skin absorption thin and inconsistent, concluding that claims about topical magnesium efficacy were not well supported by the available science. Related laboratory research measuring magnesium ion movement across intact skin found permeation was minimal, representing a small fraction of a percent of the applied dose over 24 hours, with much higher absorption only when the skin’s outer barrier was artificially disrupted or already compromised, as with broken, shaved, or otherwise damaged skin, conditions that don't describe a typical bath.
Human data on this specific question is limited but not entirely empty. One small pilot study, published in PLOS One in 2017, applied a magnesium cream and did find a modest rise in serum magnesium compared with a placebo cream, but the result reached statistical significance only in a subgroup of non-athletes, not the full sample of participants. That is a genuine, if narrow and preliminary, data point suggesting some limited absorption may be possible under certain conditions, not proof that a standard Epsom salt soak reliably changes blood magnesium levels, let alone that it goes on to measurably lower cortisol.
None of this means an Epsom salt bath does nothing for how you feel. Warm water immersion on its own is associated with muscle relaxation, reduced perceived stress, and activation of the parasympathetic nervous system, the "rest and digest" counterpart to the stress response, independent of anything dissolved in the water. Setting aside twenty minutes to sit still in warm water, away from screens, notifications, and obligations, plausibly lowers stress through that mechanism alone, which is a real and reasonably well-supported effect in its own right.
What is proven: magnesium has a real, if modest, relationship to stress hormone regulation when consumed through diet or supplements, and warm bathing itself is relaxing. What is trend and marketing: the specific claim that Epsom salt dissolved in bathwater meaningfully raises your body’s magnesium levels or measurably lowers cortisol. The more honest explanation for why an Epsom salt bath feels good is likely the bath itself, the warmth, the stillness, and the ritual, not necessarily the salt dissolved in it.
