Omega-3 Fatty Acids and Cortisol: What the Research Actually Shows

Fish oil has been a supplement-aisle staple for decades, mostly sold on heart and brain health, but omega-3 fatty acids have picked up a newer pitch in the last few years: as a way to blunt cortisol and calm stress-related skin issues at the same time. The appeal makes some sense, since omega-3s are genuinely anti-inflammatory and skin conditions like acne, eczema, and premature aging all have inflammatory components. Whether taking fish oil meaningfully changes your cortisol levels, and whether that translates into visibly calmer skin, are two separate questions the research answers with more nuance than the marketing suggests.

The mechanistic case starts with EPA and DHA, the two main omega-3s found in fish oil, which are precursors to specialized pro-resolving mediators, signaling molecules that help wind down inflammation rather than just blocking it. Because the hypothalamic-pituitary-adrenal (HPA) axis, the system that governs cortisol release, is sensitive to systemic inflammation, some researchers have proposed that omega-3s could indirectly support healthier cortisol regulation by lowering the inflammatory load the HPA axis is responding to. A 2018 analysis of blood samples from the Netherlands Study of Depression and Anxiety, covering roughly 2,700 participants, found that higher circulating omega-3 levels were associated with lower evening cortisol, a correlational finding consistent with that mechanism, though correlation alone cannot show that omega-3 intake caused the difference.

Interventional evidence is more limited but points the same direction. A randomized, placebo-controlled trial published in Psychoneuroendocrinology in 2019 gave omega-3 supplements to adults reporting occupational burnout and found that, compared with placebo, the omega-3 group showed reduced burnout symptoms and lower morning cortisol secretion after eight weeks. That is a genuinely useful data point because it is a controlled intervention rather than an observational association, but it is also a single, relatively small trial in a specific population (people already experiencing burnout), so it does not establish that omega-3 supplementation reliably lowers cortisol in everyone.

The skin side of the story runs on a more direct mechanism than the cortisol angle. Omega-3s support the skin's lipid barrier, help regulate keratinocyte turnover, and have been studied, in systematic reviews of both oral and topical formulations, for reducing inflammation in conditions like atopic dermatitis, psoriasis, and acne, along with easing UV-related redness. That barrier-support and anti-inflammatory effect on skin is fairly well documented, even though it is largely independent of whatever is happening with circulating cortisol. In other words, omega-3s plausibly help stress-prone skin through their own direct anti-inflammatory action on skin tissue, not necessarily because they are meaningfully lowering your cortisol levels first.

It is worth being honest about the limits here too. Reviewers of the topical and oral skin research consistently note that human trials are still relatively few, often small, and use inconsistent doses and formulations, which makes it hard to say exactly how much omega-3 intake, or which form, produces a noticeable skin benefit for a given person. Supplement marketing tends to skip past that uncertainty and present omega-3 as a settled fix for both stress hormones and skin inflammation, when the honest picture is a plausible, partially supported mechanism rather than a guaranteed outcome.

What is proven: omega-3 fatty acids are anti-inflammatory, support skin barrier function, and at least one controlled trial found they lowered morning cortisol in people with burnout symptoms. What is trend and marketing: the leap from those findings to omega-3 as a reliable, general-purpose cortisol-lowering or skin-clearing supplement for anyone who takes it. The current evidence supports omega-3s as a reasonable, low-risk addition to a broader routine, not a targeted fix for "high cortisol" skin.
