Weighted Blankets and Cortisol: What the Sleep Research Actually Shows

Weighted blankets are sold today with a specific physiological promise attached: that the pressure of the blanket lowers cortisol along with helping you fall asleep. It is a claim repeated across enough product pages and sleep-wellness articles that it can start to sound settled. The research that has actually measured cortisol directly tells a more complicated story.
The proposed mechanism is called deep pressure stimulation, the idea that firm, evenly distributed pressure across the body activates the parasympathetic nervous system, the body's rest-and-digest counterpart to the fight-or-flight stress response. In theory, that activation should be reflected in lower cortisol and higher levels of calming signals like serotonin and melatonin, and this mechanism is the basis for most of the "lowers cortisol" language used in weighted blanket marketing.
The most direct test of that specific claim comes from a 2023 crossover study in the Journal of Sleep Research. Researchers had 26 healthy young adults sleep under either a weighted blanket, at about 12 percent of body weight, or a light blanket, and measured saliva every 20 minutes before and during the night to track melatonin, oxytocin, cortisol, and sympathetic nervous system activity. The weighted blanket produced a clear, statistically significant result for one hormone: pre-sleep melatonin rose about 32 percent compared with the light blanket. But the same study found no significant difference in cortisol, oxytocin, or sympathetic nervous system markers between the two conditions, directly undercutting the specific claim that a weighted blanket lowers cortisol.
A separate, larger study adds a different kind of evidence. A pilot randomized controlled trial published in BMC Psychiatry in late 2024 followed 102 adults with clinically diagnosed insomnia across three hospitals in China for one month, comparing a weighted blanket to a standard one. The weighted blanket group showed significantly better scores on the Pittsburgh Sleep Quality Index, along with significant improvements in self-reported stress, anxiety, daytime sleepiness, and fatigue. That is a real, replicated benefit, but it is a subjective, questionnaire-based measure of stress, not a hormone reading, and it is a meaningfully different kind of evidence than the direct saliva-cortisol result above.
Put together, the two studies point in a consistent direction that just does not match the marketing. What is reasonably well supported: weighted blankets appear to genuinely increase melatonin and, in people with diagnosed insomnia, measurably improve self-reported sleep quality, stress, and anxiety over weeks of use. What is not supported by the best current direct evidence: that this translates into a measurable drop in cortisol. The one study built specifically to test that question found no effect.
None of this makes a weighted blanket a bad purchase. Improved subjective sleep quality and reduced self-reported anxiety, especially for people with diagnosed insomnia, are legitimate, evidence-linked benefits on their own. But "cortisol-lowering" is a more specific, physiological claim than "helps me feel calmer at night," and it is the one the direct hormone data does not currently back up. Anyone buying a weighted blanket specifically to lower cortisol is, based on the best available research, buying it for the wrong reason, even if it still helps them sleep.
